BASIC EDUCATION UNIT

ADMISSION PROCEDURES

Pre-School

Eligibility:

• 3 years old for Nursery on or before October 31
• 4 years old for Kindergarten 1 on or before October 31
• 5 years old for Kindergarten II on or before October 31

Admission Requirements:

• Original and photocopy of NSO / PSA Birth Certificate
• Two copies of recent identical 2×2 ID picture (cut-out and computer-generated pictures will not be accepted)
• Accomplished Application Form
• Parent’s Interview
• Entrance Examination
• Two (2) long brown envelopes

Grade 1

• Original and photocopy of NSO / PSA Birth Certificate
• Original and photocopy of Baptismal Certificate
• Two recent 2x2 ID pictures (cut-out and computer-generated pictures will not be accepted)
• Photocopy of current report card showing the most recent grades with Learner Reference Number
• Two (2) long brown envelopes

Grade 2 - Grade 6

• Original and photocopy of NSO / PSA Birth Certificate
• Two recent 1x1 ID pictures (cut-out and computer-generated pictures will not be accepted)
• Photocopy of current report card showing the most recent grades with Learner Reference Number
• Certificate of Good Moral Character (for transferees)
• Two (2) long brown envelopes

Grade 7 - Grade 10

• Accomplished Assessment Form
• Admission interview for New Students (as scheduled by the Principal)
• Original and photocopy of NSO / PSA Birth Certificate
• Two recent 1x1 ID pictures (cut-out and computer-generated pictures will not be accepted)
• Photocopy of current report card showing the most recent grades with Learner Reference Number
• Original Copy of Good Moral Character (incoming Grade 7 or Transfer-In)
• Conduct Grade - not lower than 80%
• Must pass the admission tests
• Two (2) long brown envelopes

Grade 11 - Grade 12

• Original and photocopy of NSO / PSA Birth Certificate
• Two recent 2x2 ID pictures (cut-out and computer-generated pictures will not be accepted)
• Photocopy of current report card showing the most recent grades with Learner Reference Number and Grade 10 Certificate of Completion
• Original Copy of Good Moral (for incoming Grade 11 and Transfer-In)
• Conduct Grade - not lower than 80%
• ESC Certificate for Grade 10 ESC Completer
• Automatic SHS VP grant to Public G10 Completer - submit one (1) copy of G10 Certificate of Completion from Public School
• Must pass the admission tests
• Two (2) long brown envelopes

Foreign Pupils/Students

• English Translation of Report Card
• Two recent 2x2 ID pictures (cut-out and computer-generated pictures will not be accepted)
• Photocopy of Passport
• PEPT (Philippine Education Placement Test) as required by DEPED

ENROLMENT PROCEDURE

For New Students

1. Accomplish the form
2. Submit the requirements together with the accomplished Enrolment Assessment Form to the office of the College Registrar
3. Pay the registration fee to finalize your enrolment
4. An official receipt will be given to you
5. Proceed to the Anthonian Virtu-Techs office for ID application

The following guidelines must be strictly followed:

• Students must wear a white blouse (girls) / white polo (boys)
• Proper haircut (2”X3” for boys) and no fancy hair color
• No dangling earrings for girls

For Old Students

1. Accomplish the Enrolment Assessment Form
2. Pay the registration fee to finalize your enrolment
